New Patient Information

 
  Welcome to Columbia Cardiology. We are pleased that you and your primary care physician have chosen us for your cardiology care. If you have an upcoming appointment, the following information will help make your first visit as comfortable as possible. If you are scheduled for an office procedure, please also see our Tests and Procedures section for further information. Finally, if you need to make an appointment, please call (503) 297-6234 for our Portland office, (503) 554-1187 for our Newberg office, or 503-216-1650 for our Hillsboro office.  
     
 
  • In an attempt to provide a smoother and timelier check-in, you will receive a packet of forms by mail prior to your visit. Please fill them out completely and bring them with your insurance cards to your first appointment. If for any reason you do not have these forms, you can download and print them now.

  • Please bring all of your medications that you are currently taking in their original bottles.
     
  • We have already requested pertinent medical records from your referring provider. If you have any additional medical records, pictures, device information, etc. that are pertinent to your cardiovascular history, please bring them with you to your appointment.
     
  • Your insurance plan may require a referral. In order to assist you, we have requested a referral from your primary care physician. We ask that you contact your primary care physician's office to verify the referral has been completed prior to your appointment. We do have patients sign a waiver indicating they will be responsible  for payment for the service if the referral has not been received.
 
  Finally, you can expect a friendly staff and comfortable environment where you receive individual attention and our expert opinion. You can be satisfied that we will fulfill our duty in fully explaining your diagnoses, test results, need for further testing and treatment plan.
