• Please bring your completed forms and insurance cards with you.
   
• Please bring all of your medications that you are currently taking in their original bottles.
   
• If you have any additional medical records, pictures, device information, etc. that are pertinent to your cardiovascular history, please bring them with you to your appointment.
   
• Your insurance plan may require a referral. In order to assist you, we have requested a referral from your primary care physician. We ask that you contact your primary care physician's office to verify the referral has been completed prior to your appointment. We do have patients sign a waiver indicating they will be responsible  for payment for the service if the referral has not been received.
